Use

This cluster is used to document the professional assessment of the validity of a clinical record.

Typical use cases include:

  • A healthcare professional records an immunization, but it is ultimately not administered, and the record must be marked as incorrect and removed.
  • A previously recorded dose is later identified as not having been administered and is therefore considered an erroneous entry.
  • Clinical data quality processes, audits, or record validation workflows where incorrect entries must be identified and justified.
Misuse

This cluster should not be used for:

  • Representing whether a clinical act (e.g. immunization, medication administration) has been performed or not. This must be recorded in the appropriate clinical archetype (e.g. ACTION).
  • Replacing workflow or status attributes such as “completed”, “cancelled”, or “not done”.
  • Capturing patient clinical conditions or outcomes.
  • Performing automatic system validations without professional assessment.

This cluster is strictly intended to evaluate the correctness of a record, not the execution status of a clinical activity.

PurposeThis cluster represents the validation of a clinical record, indicating whether the recorded information is correct or incorrect, together with the justification for that assessment and any additional contextual details. It is particularly intended to identify records that were entered in error and do not correspond to an actual clinical event (e.g. a medication or immunization recorded but not administered).
